Office Information

We value you as a patient and are completely committed to providing you with the highest standards of medical care!!  Below you will find information about our policies, payment, and patient forms.  Please contact our office at 919-553-3900 if you need any additional information or have any questions about our policies.

Office Hours

Our office hours are from 8:00 to 5:00 Monday – Friday

In the event of a medical situation that occurs after office hours, over the weekend or on a holiday; our physician on call can be reached through the Johnston Memorial Hospital switchboard at (919) 934-8171. Occasionally we do share call with other physicians in the community. You will never be left without the ability to contact a physician.

In the event of a life threatening emergency, please dial 911 immediately or present to the Emergency Department at either Johnston Memorial Hospital or WakeMed Raleigh.

Billing / Payment

We participate with most insurance carriers and we will automatically file primary and/or secondary insurance claims with those companies.

Your insurance company requires our office to collect co-payments, deductibles, and coinsurance amounts at the time of service.  If you are unable to make your payment according to the terms of your insurance policy at time of check-in, you will not be seen by the provider.

Self-pay patients, or patients who pay the entire bill for services incurred in this office, are expected to pay in full at the time of service.  All MVA related visits are treated as self-pay as we do NOT file any claims for these types of visits.

Our staff will verify your demographic information (address/telephone numbers) and ask to see your insurance cards at the time of every visit in order to be certain that all of our records are kept accurate and up to date.

Forms of payment accepted by the office are cash, personal checks, money orders and the following debit and credit cards. There is a $25 processing fee for any checks that are returned unpaid.

Medical Records

We require pre-payment for copying medical records and a signed medical records release form. Copying fees for medical records is seventy-five cents ($0.75) per page for the first twenty-five (25) pages, fifty cents ($0.50) per page for pages 26 – 100, and twenty-five cents ($0.25) for each page in excess of 100 pages.

If you are unable to keep a scheduled appointment, we request that you notify us at least 24 hours in advance. This courtesy, on your part, makes it possible for our scheduling staff to reallocate the time that had been set aside for you to another patient. Our staff will be able to either cancel or reschedule your appointment. We may charge a “no show” fee for patients that disregard this request.

While we do our very best to see patients at their scheduled appointment times, occasionally there are circumstances beyond our control which may place a provider behind schedule. Because it is our ultimate goal to provide each and every individual patient with quality care, we appreciate your understanding and cooperation – especially during the busier times of year.

We also realize that your time is valuable, so we would appreciate it if you notify any member of our staff if you have been waiting more than 20 minutes beyond your scheduled appointment time.

Patients that arrive late for their appointments may be asked to reschedule the appointment.
